- Cardboard sleeve with blue dots and black text holds a hinged, white, cardboard box. Box contains one rubber diaphragm marked "ortho / 80." The box has small ventilation holes on the bottom. Box includes a stapled booklet, "Directions for Use and Care of the Diaphragm" which includes illustrations. Text on box sleeve: "ortho / diaphragm / PACKAGE F . . . DIAPHRAGM -- Distributed by / ORTHO PRODUCTS, INC., LINDEN, NEW JERSEY MADE IN U.S.A. / CAUTION: To be used only by or on the prescription of a physician." The "ortho" trademark appearing on this package was registered April 29, 1941; claimed use since Nov 28, 1940. Reg No. 387,080.
